Your medical certificate is your lifeline. Without a valid DOT medical card, you cannot legally drive a commercial vehicle. In 2026, the FMCSA rolled out two significant changes that every Class A driver needs to understand.

Change 1: Certified Medical Examiner Registry Verification

As of January 2026, all DOT physicals must be performed by a medical examiner listed on the FMCSA National Registry. The ME must enter your results electronically within 24 hours. No more paper-only certificates. If your doctor is not on the registry, your physical is invalid.

Change 2: Sleep Apnea Screening Protocol

The FMCSA now requires medical examiners to screen for Obstructive Sleep Apnea (OSA) using the STOP-BANG questionnaire plus BMI calculation. If your BMI is 35+ OR you score 5+ on STOP-BANG, the ME must refer you for a sleep study before certifying you.

What This Means for You:

  • If you are overweight, get ahead of this. Start a weight management program now.
  • If you already use a CPAP, bring your compliance data (at least 4 hours/night, 70% of nights) to your physical.
  • Budget $300 to $800 for a sleep study if referred. Some insurance plans cover it.

What If You Fail?

You are not necessarily done. Many conditions can be managed. Diabetes controlled with medication (not insulin) is certifiable. High blood pressure can be managed with meds. Heart conditions require a cardiologist clearance letter. The key is preparing BEFORE your appointment, not scrambling after a failure.

Pro Tip:

Schedule your physical 30 days before your current certificate expires. If something goes wrong, you have time to fix it without going out of service.
